Summary | The haemodynamic changes induced by Streptokinase (SK) infusion in patients with Acute Myocardial Infarction (AMI) and normal haemodynamic state were studied in 58 patients (pts). A drop in arterial pressure affecting both the systolic and diastolic blood pressure was noticed in the 53 of the 58 pts (93%). Systolic Blood Pressure (SBP) fell from 142 mmHg to 95 mmHg and Diastolic Blood Pressure (DBP) from 85 mmHg to 65 mmHg. A concomitant drop in the Pulmonary Wedge Pressure (PWP) was recorded in the subgroup of catheterised patients, which dropped from 14.1 mmHg to 8 mmHg. The haemodynamic changes were more prominent in patients with inferior infarction compared to patients with anterior infarction. The event was transient lasting less than 45 mins in all but 4 cases. Nitroglycerine infusion was interrupted in 29 cases and saline infusion was used in 29 pts. The event was considered severe in 15 cases (25.7%), in 4 of which inotropic drugs (dopamine) were used. In all patients normal haemodynamic state was reestablished and no major complications, attributable to the hypotensive effect of the drug, were noticed. | |
