Summary Glossopharyngeal neuralgia is a rare cause of orofacial pain that presents difficulties in both the diagnostic and the therapeutic field. We provide a review of the epidemiology, aetiology, diagnosis and therapeutic alternatives that are available for this condition. Depending on the cause, glossopharyngeal neuralgia may be differentiated to an idiopathic and to a secondary form. Vascular compression, tumors, local infection, multiple sclerosis and trauma are the most common causes of the secondary form. Medical treatment primarily with carbamazepin or gabapentin is considered first choice. Several other drugs have also been used. In patients not responding to medical treatment, surgery should be considered. Microvascular decompression constitutes the most popular surgical technique. Experimental treatment techniques such as percutaneous radiofrequency neurolysis and gamma knife surgery have also been used.
