Summary Severe haemolytic anaemia is a rare complication of drug induced antibody production. Cephalosporines that are widely used as chemoprophylaxis for the prevention of surgical wound infections, have been implicated in drug induced haemolytic anaemia. The present study attempted a systematic review of the literature on cephalosporin induced haemolytic anaemia in surgical patients in order to increase awareness of this rare adverse event and to help clinicians in the diagnosis and treatment of this entity. Twenty three cases of cephalosporin induced haemolytic anaemia in surgical patients were identified, including a fatal one. Cefotetan was implicated in the majority of cases.
