Summary In order to define the role of haematopoietic stem cells (HSC) in the evolution of septic syndrome caused by ventilator-associated pneumonia (VAP) the alterations of the expression of CD34 a prospective study was designed with 43 patients with septic syndrome by ventilator-associated pneumonia. Blood was sampled on day of enrollment and on days 3, 5, and 7 thereafter and percentage and absolute values of CD34 and concentrations of tumour necrosis factor-alpha, interleukin-6 (IL-6), IL-8 and malondialdehyde (MDA) were estimated. No differences were found either in percentage and absolute numbers of CD34, or in concentrations of TNFa, IL-6, IL-8, and MDA among patients with sepsis, severe sepsis and septic shock. No differences were found between the concentrations of TNFa and MDA in the days sampled, whereas CD34 percentage was elevated on day 7 compared to day 1, IL-6 on days 1 and 3 was lower than day 7, and IL-8 was higher in day 5 than day 1. A positive correlation was also revealed between IL-6 on day 1 and CD34 percentage on day 5, between CD34 percentage on day 1 and IL-6 on day 7, and between absolute CD34 count on day 1 and IL-8 on day 3. It is concluded that CD34 HSC play a significant role in the evolution of sepsis by VAP by regulating production of white blood cells and of cytokines. In addition, CD34 expression is influenced by the levels of cytokines in blood.
