Summary The evidence that vitamin A may protect against lung cancer is reviewed. Retinol (pre-formed vitamin A) and related compounds (retinoids) have long been known to have properties useful for the inhbition or even the reversion of cancer in experimental animals and in in vitro systems. They promote cell differentiation, modulate cell growth and inhibit experimental chemical carcinogenesis. However, most epidemiological studies (both dietary surveys and prospective or retrospective measurements of serum retinol levels) have so far failed to produce any convincing evidence of a protective effect against cancer in man. On the other hand, epidemiological studies have consistently shown an association of high consumption of β-carotenerich foods (primarily green and yellow-green vegetables) with a small but significant reduction in lung cancer rates. Experimentally little is known about the effects of β-karotene on carcinogenesis. It is possible that, by efficiently quencing endogenously produced singlet oxygen and other excited oxygen species (for which accumulating evidence suggests an involvement in the later stages of carcinogenesis), β-carotene may inhibit one or more stages of tumour promotion. If this is so it may be anticipated that intervention studies with β-carotene may yield observable results in a relatively short time.
