Summary Etomidate infusions have been associated with increased morbidity and mortality due to inhibition of adrenal steroidogenesis, as in vitro studies have demonstrated. Since bolus administration of various anesthetics has been proven as effective, the aim of our study was to evaluate the efficacy of three such agents and assess the safety in solar glucocorticoid secretion in vivo. Our study was carried out in 20 women undergoing total hysterectomy. Etomidate (E) 0.3 mg/kg, midazolame (M) 0.2 mg/kg or thiopental (T) 5 mg/kg were given as in i.v. bolus. Plasma cortisol was measured in blood samples collected prior to the induction and serially up to 2 hour thereafter. Our findings documented that neither of these three anesthetics administered as a bolus resulted in suppression of glucocorticoid production and it is thus recommended over the infusion for safety purposes.
