Summary Among the autoimmune disorders, chronic lymphocytic thyroiditis, usually well known as Hashimoto’s thyroiditis, shares a unique and distinguished entity between the autoimmune disorders and the inflammatory diseases of the thyroid gland. Chronic thyroiditis or Hashimoto’s disease is a common thyroid gland disorder that can occur at any age, but it is most often seen in middle aged women. Hashimoto’s thyroiditis represents the one end of a disease spectrum where an immune response is directed against the thyroid gland and causes cell-mediated and humoral thyroid injury leading to destruction of thyroid tissue and thyroid hormone deficiency. The mechanism by which this disease develops is unknown, although it is believed that the disease occurs in genetically susceptible individuals exposed to a permissive environment. A number of environmental factors have been postulated to be involved in the development of Hashimoto’s thyroiditis. There is, however, no direct evidence to support clear causality. Susceptibility loci within immune response genes have been identified although a significant component of the genetic predisposition to disease remains unknown. This review, presents a comprehensive overview of the literature first on pathogenesis and clinical and laboratory manifestations, and discusses the complications, differential diagnosis and treatment of the disease. The association of chronic lymphocytic thyroiditis with other diseases of autoimmune origin and the presence of malignancies involving the thyroid gland, as well as extra-thyroid tissues, are also described.
