Summary Tonsillitis, a minor disorder, can result in Lemierre’s syndrome, a life-threatening condition caused by Fusobacterium necrophorum. We report a new case of a previously healthy young female, who presented with high fever and tonsillitis with ipsilateral cervical lymphadenitis, progressing towards pneumonia. Blood cultures, which became available during the second week of treatment, specified the causative pathogen. This condition should be included in the differential diagnosis of patients with tonsillitis, persistent high fever and pulmonary symptoms. In our case, the patient responded well to the combination of metronidazole and imipenem.
