Summary | The aim of the present study was to compare the status of Chemoprophylaxis in General Surgery Departments of two University Hospitals in Denmark and in Greece, and to check if general guidelines for Surgical Chemoprophylaxis are met. The Department of General Surgery of the West Zealand Hospital of Slagelse (Denmark) and the 5th University General Surgery Clinic of the Hippokrateion Hospital of Thessaloniki (Greece) was chosen for the study. The Danish and Greek surgeons replied to the same questionnaire, which investigated the conformance to the recommended guidelines and the behavior of the surgeons in Surgical Chemoprophylaxis in clean and contaminated surgery, and in patients having a low or a high risk for a post-operative infection. The following issues in Chemoprophylaxis were checked: 1) application of chemoprophylaxis, 2) duration and time of initiation of chemoprophylaxis and 3) the kind of antibiotics used. According to the guidelines, neither the Danish nor the Greek surgeons used chemoprophylaxis in low risk patients undergoing a clean surgery. In high risk patients undergoing a clean surgery, the Danish surgeons used chemoprophylaxis in most cases, while the Greek surgeons used chemoprophylaxis in all cases. In contaminated surgery, the Greek surgeons always used chemoprophylaxis, while the Danish surgeons did not use chemoprophylaxis in many operations, when the patients had a low risk for a post-operative infection. Chemoprophylaxis was almost always initiated during the induction of anesthesia and its duration was usually shorter in Denmark than in Greece. Both Danish and Greek surgeons used a β-lactam for chemoprophylaxis but the Danish surgeons used also gentamicin in esophagus and colon surgery. Metronidazole was also used in cases that an anaerobic contaminant was suspected. In conclusions, non-conformance to the guidelines was sometimes observed in both Greek and Danish surgeons’ behavior on peri-operative chemoprophylaxis. | |
