Περίληψη – Summary

O καρκίνος του μαστού αποτελεί ένα χρόνιο νόσημα με υψηλή θνησιμότητα. Συνεπώς η πρόληψή του  είναι ιδιάζουσας σημασίας. Σκοπός της παρούσας ανασκοπικής εργασίας ήταν να διερευνηθεί η σχέση  καρκίνου του μαστού με την διατροφή. Η μεθοδολογία που ακολουθήθηκε περιελάμβανε αναζήτηση σε ηλεκτρονικές βάσεις δεδομένων (Pubmed, Scopus), ανασκοπικών και ερευνητικών μελετών με λέξεις κλειδιά (breast cancer, nutrition, obesity, physical activity, prevention, treatment), που εκπονήθηκαν σε εθνικό και διεθνές επίπεδο και αναφέρονταν στο καρκίνο του μαστού και την συσχέτιση αυτού με τη διατροφή. Κριτήριο αποδοχής δεδομένων ήταν όλες οι έρευνες, οι οποίες μελέτησαν την επίδραση των διατροφικών συνηθειών στον καρκίνο του μαστού των γυναικών, ενώ τέθηκε χρονολογικός περιορισμός εικοσαετίας ώστε να συμπεριληφθούν παλαιότερες μελέτες, καθώς και οι πιο πρόσφατες και ενημερωμένες έρευνες που αφορούν την συγκεκριμένη επιστημονική περιοχή. Η διεθνής βιβλιογραφία τις δύο τελευταίες δεκαετίες καταδεικνύει, ότι το ποσοστό επιβίωσης γυναικών με καρκίνο του μαστού αυξήθηκε, ως αποτέλεσμα της έγκαιρης διάγνωσης και της αυξημένης χρήσης της συμπληρωματικής χημειοθεραπείας. Πλήθος ερευνητικών μελετών έδειξαν αιτιολογική συσχέτιση της διατροφής με τον καρκίνο του μαστού, αλλά επίσης μεγάλος αριθμός ερευνών έδειξε ότι κάποια διατροφικά στοιχεία δρουν προστατευτικά. Ωστόσο, τα ευρήματα που υποστηρίζουν την επίδραση της διατροφής ή συγκεκριμένων συστατικών με επιβαρυντική ή προστατευτική δράση στον καρκίνου του μαστού δεν είναι αδιάσειστα. Συμπερασματικά, είναι απαραίτητες περαιτέρω έρευνες σχετικά με την επιδημιολογική θεώρηση της νόσου, τη Φυσιολογία της και τους παράγοντες κινδύνου (γενετικούς, διατροφικούς, περιβαλλοντικούς). Η πολύπλευρη σημασία της σωστής διατροφής στην πρόληψη του καρκίνου του μαστού, θα πρέπει να αποτελεί αναπόσπαστο μέρος των στρατηγικών πρωτοβάθμιας πρόληψης, αλλά και κάθε στοχευμένης παρέμβασης για την έγκαιρη διάγνωση και την αποτελεσματική θεραπεία.

Τhe relationship between the fat tissue, the inflammation and the metabolic syndrome,  by means of the broad acceptance of such term, is presented in this review article. Inflammatory products induce an insulin resistance which in turn favors, by means of nutrition, the activity and the structure of the cells that participate in the inflammatory process. Fat tissue, having a genetic relationship with the immune system, participates in the manifestation of the metabolic syndrome by contributing to the secretion of inflammatory cytokines. Insulin resistance, blood vessels inflammatory processes and lipid synthesis are controlled by a variety of mechanisms. Such mechanisms like the endogenous cannavinoid system, the endogenous opiate system, the adrenergic system, PPARs receptors and the local glycocorticoid metabolism in the fat tissue were reviewed in an attempt to demonstrate combined anti-inflammatory and lipoanabolic procedures. Finally, the example of vitamin D deficiency as the reason of a reversible angiotropic inflammation was analyzed.
