Περίληψη – Summary When a woman becomes pregnant huge and progressive functional changes in every system may produce major effects on the pharmacodynamics and pharmacocinetics of any drugs she may be given. Drug absorption is affected negatively by the much slower gastric emptying and the complexation of the drug to commonly used antacids, and positively by the increased respiratory activity, pulmonary blood flow, blood flow in the skin, in the vaginal mucous membrane and in the epidural space. Drug distribution might be modified during pregnancy as a consequense of the progressive increasing plasma, total blood and body water volumes (much enlarged distritution volume). Protein binding of drugs changes due to decreased albumin concentration (30%) and to accumulation of endogenous inhibitors of drugs protein binding. Drug metabolism may be altered in pregnancy, particularly as a consequence of induction of microsomal drug metabolising enzymes by other xenobiotic substances (progesterone, cigarette smoking). Excretion ‘of drugs is enhanced by a increase in renal filtration and the reduce of protein binding. The global effect of these changes is to reduce the plasma concentration of a drug. On the other hand, the embryo constitutes one of the most dynamic biological systems, characterized by continuous cellular changes. The embryo in utero is sensitive to harmful actions of dysmorphogenic agents, physical factors – X rays and anoxia -, viral infections, endotoxins, chemicals-drugs). A drug will have an dysmorphogenic or embryotoxic effect when it is given at an appropriate dosage, at a very precise moment during the morphogenesis of a genetically susceptible embryo. Drugs should be used in pregnancy if they are clearly indicated and of proven  benifit to the mother or fetus. Drugs proven to cause congenitcal abnormalities are: androgens, anticonvulsants, cytotoxic drugs, thalidomide, virilising progestagens and warfarin.
