Περίληψη – Summary
Η αποτυχία της θεραπείας με αντιβιοτικά είναι σημαντικό πρόβλημα στην κλινική πράξη. Αν και το φαινόμενο αυτό δεν φαίνεται να είναι συχνό, εν τούτοις πρέπει να τονιστεί ότι υπάρχουν πολλές πιθανές αιτίες αποτυχίας της θεραπείας με αντιβιοτικά. Οι συνηθέστερες αιτίες είναι η λανθασμένη διάγνωση, μη θεραπεύσιμες λοιμώδεις νόσοι, μη καλή γνώση των φαρμακοκινητικών/φαρμακοδυναμικών ιδιοτήτων του αντιβιοτικού, η ανάπτυξη αντοχής και άλλα. Ένα από τα πλέον συχνά σφάλματα  που γίνονται σε περιπτώσεις αποτυχίας της θεραπείας με αντιβιοτικά είναι η αλλαγή ή προσθήκη αντιβιοτικών. Ευρισκόμενοι ενώπιον μιας αποτυχημένης θεραπείας με αντιβιοτικά επιβάλλεται η ανάλυση της αιτίας της αποτυχίας με προσεκτική εκτίμηση και τη χρήση των κατάλληλων διαγνωστικών εξετάσεων για να αποφευχθεί η άνευ λόγο εφαρμογή δαπανηρής και, ενίοτε, δυνητικά επικίνδυνης αντιμικροβιακής θεραπείας.
With the availability of modern antibi­otics, many of which have extremely broad anti­bacterial spectrum, and with adequate knowledge of their pharmacoki­netic and pharmacodynamic characteristics, thera­peutic failure is uncommon. There are many possi­ble causes of antibiotic drug failure, but the most common untreatable infectious diseases, noninfec­tious diseases, infectious dis­eases requiring sur­gery, drug fevers, or problems with incorrect or inadequate spectrum of antibiotics, super infec­tious, emergency resistant organisms, and drug interactions. The most common mistake made with apparent antibiotic failure is to change or add addi­tional antibiotics. To avoid antibiotic failure we must analyze the cause by careful evaluation and use of appropriate diagnostic tests and not to prescribe expensive and potentially dangerous antibiotics.
