Περίληψη – Summary

Ο Παγκόσμιος Οργανισμός Υγείας (ΠΟΥ) εκτιμά ότι η χρήση καπνού αποτελεί τον πρωτεύοντα παράγοντα κινδύνου που σχετίζεται με την πρόωρη θνησιμότητα στην Ευρώπη, προκαλώντας περίπου 1,3 εκατομμύρια θανάτους ετησίως. Ο Αμερικανικός Ψυχιατρικός Σύλλογος ορίζει την εξάρτηση από το κάπνισμα ως την εξάρτηση από τη νικοτίνη, ενώ ο ΠΟΥ (ICD-10) ως σύνολο από συμπεριφορικά, γνωστικά και ψυχολογικά φαινόμενα που εκδηλώνονται μετά από επανειλημμένη χρήση, συμπεριλαμβάνοντας τυπικά την ισχυρή επιθυμία για κάπνισμα, τη δυσκολία να ελεγχθεί η χρήση, την επιμονή στη χρήση παρ’ όλες τις επιβλαβείς συνέπειες του καπνίσματος και την αυξημένη ανοχή στη νικοτίνη. Η επιβεβλημένη ανάγκη διακοπής του καπνίσματος συντέλεσε στην εύρεση διαφόρων μεθόδων αποχής από το κάπνισμα, οι οποίες ταξινομούνται σε δύο κατηγορίες. Η πρώτη κατηγορία περιλαμβάνει τις μη φαρμακολογικές μεθόδους, ενώ στη δεύτερη κατηγορία συγκαταλέγονται φαρμακολογικές παρεμβάσεις και συγκεκριμένα η θεραπεία υποκατάστασης με νικοτίνη, βουπροπιόνη, νορτριπτιλίνη, κλονιδίνη και βαρενικλίνη. Με αφορμή την πρόσφατη (30/6/2009) υπουργική απόφαση σχετικά με την εφαρμογή του Νόμου για τη διακοπή του καπνίσματος σε δημόσιους χώρους, η παρούσα εργασία αποσκοπεί στη σύγκριση της αποτελεσματικότητας και των ανεπιθύμητων ενεργειών των φαρμακολογικών σκευασμάτων για τη διακοπή του καπνίσματος βάσει της Ιατρικής βασισμένης στην τεκμηρίωση. Για το σκοπό αυτό αναλύθηκαν τυχαιοποιημένες κλινικές μελέτες, μελέτες κοόρτης, συστηματικές ανασκοπήσεις και μετααναλύσεις δημοσιευμένες στην Αγγλική γλώσσα μεταξύ του 2006-2009. Η πλειονότητα των εν λόγω ερευνών υποδεικνύει ότι η βαρενικλίνη είναι αποτελεσματικότερη για τη διακοπή του καπνίσματος και, σύμφωνα με πρόσφατες κατευθυντήριες οδηγίες της Υπηρεσίας Δημόσιας Υγείας των ΗΠΑ, προτείνεται ως πρώτης γραμμής φαρμακολογική επιλογή για τη διακοπή του καπνίσματος. Παρά ταύτα, επισημαίνονται τα μειονεκτήματα των διαθέσιμων ερευνών, τα οποία υποδεικνύουν ότι είναι απαραίτητος ο σχεδιασμός και η πραγματοποίηση επιπρόσθετων ερευνών, ώστε να ελεγχθεί η αποτελεσματικότητα και η ασφάλεια της βαρενικλίνης σε δείγμα καπνιστών με ποικίλες παθολογικές καταστάσεις που ανταποκρίνονται στο γενικό πληθυσμό.

World Health Organization (WHO) estimates that tobacco use is the main risk factor related to premature death in Europe, causing around 1.3 million deaths per year. The American Psychiatric Association defines tobacco (smoking) dependence as nicotine addiction, whereas WHO (ICD-10) defines it as a cluster of behavioral, cognitive and physiological phenomena that develop after repeated use and typically include a strong desire to smoke, difficulty in controlling its use, persistence in its use despite harmful consequences and increased tolerance to nicotine. According to all recently published data, smoking cessation is crucial for health. This has led to the development of several pharmacological and non-pharmacological smoking cessation treatments. Pharmacological smoking cessation treatments include: NRT-nicotine replacement therapy, bupropion, clonidine and varenicline. Methodology: We have reviewed RCTs, cohorts, systematic reviews and meta-analysis, carried out from 2006 to 2009 that have been dealing with the efficacy and the most common adverse events of varenicline, bupropion and NRT. The majority of these data lead to the conclusion that varenicline is the most effective pharmacological agent for smoking cessation and is recommended as the first-line treatment for smoking cessation by the Health Public Services in the USA. However, several drawbacks of the available data are pointed out, which indicate that further investigation is required to confirm the effectiveness and safety of varenicline using more representative sample of smokers, as compared to the general public.
