Περίληψη – Summary
Η Νεφρική Ανεπάρκεια (ΝΑ) αποτελεί παγκόσμιο πρόβλημα υγείας με συχνότητα ασθενών που αυξάνεται συνεχώς. Η αιμοκάθαρση αποτελεί τη θεραπευτική μέθοδο αντιμετώπισης για περισσότερους από 250.000 ασθενείς με Χρόνια Νεφρική Νόσο (ΧΝΝ) στην Ευρώπη, ενώ περίπου 63.000 ασθενείς τον χρόνο εντάσσονται σε αιμοκά­θαρση στα 25 Κράτη της Ευρωπαϊκής Ένωσης Ευρώπης. Οι ασθενείς που υποβάλλονται σε αιμοκάθαρση έχουν πτωχή φυσική κατάσταση, κόπωση, καρδιαναπνευστικά προβλήματα, ψυχικές διαταραχές, σοβαρούς λειτουργι­κούς περιορισμούς και κακή ποιότητα ζωής. Στη βιβλι­ογραφία υπάρχουν αρκετές μελέτες όπου συμπεριλαμ­βάνουν την άσκηση σε ασθενείς με Νεφρική Ανεπάρκεια. Σκοπός της παρούσας εργασίας είναι η διερεύνηση της δυνατότητας εφαρμογής συστηματικών προγραμμάτων άσκησης σε ασθενείς με ΧΝΝ και η καταγραφή των πιθανών θεραπευτικών αποτελεσμάτων. Για το σκοπό αυτό πραγματοποιήθηκε βιβλιογραφική ανασκό­πηση από πηγές MEDLINE, EMBASE, Googlescholar, Cohrane. Τα κριτήρια εισαγωγής ήταν τυχαιοποιημένες ελεγχόμενες μελέτες στα Αγγλικά όπου ενήλικες με ΧΝΝ εφάρμοσαν οποιαδήποτε μορφής άσκησης για χρονικό διάστημα 8 τουλάχιστον εβδομάδων. Αποτελέσματα. Από τις  95 μελέτες εξαιρέ­θηκαν 51 που είτε δεν ήταν τυχαιοποιημένες ελεγχόμε­νες, είτε ήταν ανασκοπήσεις. Στις υπόλοιπες  43 πραγ­ματοποιούνται ασκήσεις για καρδιαναπνευστική αποκα­τάσταση, συνδυαστικά ασκήσεις αντιστάσεων και αερό­βια άσκηση, ασκησιολόγιο μόνο με αντιστάσεις, yoga. Υπάρχουν προγράμματα που πραγματοποιούνται με επίβλεψη θεραπευτή, ενώ άλλα όχι. Η διάρκεια της άσκησης ποικίλει, από 20 λεπτά έως 110. Τα αποτελέ­σματα δείχνουν πως οι ασθενείς με ΧΝΝ μπορούν να ωφεληθούν σημαντικά από τη συστηματική άσκηση. Σύμφωνα με αυτά, η άσκηση σε ενήλικες με ΧΝΝ έχει πολλά θεραπευτικά οφέλη, καθώς αυξάνει το προσδόκιμο και την ποιότητα ζωή τους. Οι ασθενείς με ΧΝΝ θα πρέπει να ενθαρρύνονται να συμμετέχουν σε προγράμματα άσκησης.

Introduction: Renal Failure (RF) is a worldwide public health problem with a constantly increasing prevalence. Hemodialysis is a therapeutic method of treatment for more than 250,000 patients with chronic kidney disease (CKD) in Europe. The numbers of new patients undergoing dialysis treatment in the 25 countries of the European Union are estimated to be 63,000 per year. Patients undergoing dialysis have poor physical fitness, fatigue, cardiorespiratory problems, mental disorders, severe functional limitations and poor quality of life. Within the literature there are several studies encompassing exercise are reported in patients with Renal Insufficiency. The aim of the present study is 1) to investigate the applicability of systematic exercise programs in patients with CKD and 2) to record the possible therapeutic effects of exercise. Methods: A systematic review of the literature using MEDLINE, EMBASE, Googlescholar, Cohrane databases was performed.  Studies were selected using inclusion and exclusion criteria. We included only those studies that: assessed exercise protocols for more than 8 weeks in adults and were randomized control trials (RCT) written in English. Results. The search resulted in 94 articles, 51 of which were not RCTs presenting the findings of 43 articles. In the remaining 43 trials exercises were performed for cardiopulmonary rehabilitation, combined resistance exercises and aerobic exercise, only resistance exercises, yoga. There were programs conducted by therapist supervision while others not. The duration of training varied from 20 minutes to 110. The results showed that patients with CKD can greatly benefit from regular exercise. Conclusions. Exercise training in adults may reduce mortality rates and improve quality of life in adult patients with chronic kidney disease. Efforts to introduce exercise training to these patients may be clinically beneficial.
