Abstract
Background: Diabetic nephropathy (DN) is a leading cause of chronic kidney disease (CKD) worldwide. Dapagliflozin has demonstrated renoprotective effects in randomized clinical trials; however, evidence from routine clinical practice in Oman remains limited. Aim: To evaluate the effect of dapagliflozin on albuminuria and renal function among Omani patients with type 2 diabetes mellitus and microalbuminuria. Methodology: A retrospective cohort study was conducted at Al Diwan Polyclinic, Muscat, Oman. Adult patients with type 2 diabetes mellitus, microalbuminuria, and at least three months of dapagliflozin therapy were included. Clinical and laboratory parameters were assessed at baseline and during follow-up. Statistical significance was set at p<0.05. Results: A total of 185 patients were included. Mean albuminuria decreased from 17.41 mg/g at baseline to 14.30 mg/g, 12.52 mg/g, and 11.77 mg/g during successive follow-up visits, representing reductions of 17.9%, 28.1%, and 32.4%, respectively. Estimated glomerular filtration rate remained relatively stable during the initial six months, then showed a modest decline during later follow-up. HbA1c demonstrated a sustained reduction throughout the study period. Conclusion: Dapagliflozin was associated with reductions in albuminuria and improvements in glycemic control among patients with type 2 diabetes mellitus and microalbuminuria. These findings support the potential renal benefits of dapagliflozin in routine clinical practice and warrant further prospective investigation.
